THE NEW ON LINE SMART CARD APPLICATION HAS BEEN FURTHER SIMPLIFIED

1.      The requirement of Email for Registration has been made optional.

2.      The Pass word configuration has been simplified to include combination of atleast eight numerals and characters.

3.      PAN Number has been made optional.

4.      Drug Allergy is to be provided if applicable and is optional.

5.      Forgot Pass word feature has been added with security aquestion.

6.      Temporary save is available to download to verify the filled up application.

Now if you want our help to apply for new Smart card, you may send the copy of the following documents (either Xerox copy or By scanning) and send it by post or Email.

We will prepare your application, submit it on line and send you the Auto Generated Temporary Slip and copy of the Registered application.

Documents to be sent:

1.      Pension Payment order.

2.      ECHS Temporary Slip or Old ECHS Cards.

3.      Photographs of all members with their names and relationship written on back side of the photograph.

4.      Signature of all members in a separate sheet with their names with sufficient gap between names.

5.      Photo should not be older than six months. 2 x 2 “ Size with white background.

6.      Download the attached “Primary application form’ correctly, sign in the form, paste the photos and send post or by scanning.
