SOME GUIDELINES FOR NEW ECHS SMART CARD

  Affidavit is not required any more.

Prefix and Suffix in the Service Number is only for Army Officers and Army JCOs. 

 Those who do not have Suffix can fill up the Alphabet ‘Z’ in the box.

    Mobile Number and Email ID of Primary beneficiary can be entered for the 

dependents for whom mobile number and email ID is not available.

    Discharge Book not required to be scanned and submitted.

      After uploading all the documents, the Status of the application can be checked

 by in your Log in.

      The life of the smart card is minimum 10 years.  However, same can be continued 

depending on the condition of the card.

     The hard copy of the application related to the application such as PPO Copy, 

Old Temporary slip, Self attestation proforma etc. etc. need not be submitted 

to any office. But keep it with you safe.

      The temporary slip will be auto generated only for the first time applicant

 of ECHS on successful submission of application.  

The temporary slip can be activated from the parent polyclinic.

      Your record office will intimate you about the status of your application

 either by SMS or Email.  Therefore, keep in touch with the person 

who has helped you to apply for the Smart card with your 

Email ID and Pass word.  Also time to time watch your 

mobile phone for any message from your Record Office.  

This is very important.

 Finally you have to personally collect the new smart card from your Station HQ.

 I think the Cost of the Smart card has been reduced from 

Rs.177 to Rs.132.50(inclusive of all taxes) plus transaction charges extra 

(as applicable) I came to know while applying for my Card and few of my friends.

   No charges, if you pay for the cost of Cards through SBI Debit Card. 

 But if you  pay through SBI Net banking, the charges are Rs.11.80 for each transaction.

  Be careful, the same SBI in other modes charges Rs.59/-.  I do not know how ?

 The ECHS website has not informed the changes in the cost of Smart Cards so far.

 Note: Those who are badly in need of Smart Card for emergency treatment etc.,

 or those who have lost their cards need to apply urgently.  

Others can wait for few days to apply for the card as some 

relaxations/changes are expected to come.

There is no instructions for those who have not joined the ECHS so far.

All the instructions are only for the present members.

The ECHS should give instructions for the Pre 1996 New applicants.
