ON LINE ECHS SMART CARD APPLICATION – SEE HOW EASY IT IS
(Launched w.e.f. 14.01.2018)

The following documents are mandatory for applying ECHS smart card online.

1.      Your old Smart Card.
2.      Your PAN Card is a must. (if you do not have, please get one immediately)
3.      You must have a Mobile No.
4.      You must have an email id.
5.      An affidavit from notary public for dependents.
6.      Affidavit is required for loss of smart card.
7.      Neat and clean photo color 2” x 2 “ size with white background taken within six months.
Purchase a Pen Drive (Good one cost around Rs.500) Scan the following documents and store in your Pen Drive and keep ready for online application.
a)      Scan your Discharge book with all important particulars. The size should be within
1 mb.
b)      Scan your PPO and store.
c)      Death certificate of ESM (Only in case of Family ECHS member)
d)      Disability Medical certificate. (As applicable)
e)      Old Temporary Slip you are using for treatment.
f)       Affidavit for dependent.
g)      Affidavit for loss of Smart Card.
h)      Birth certificate/Matriculation certificate (In case of dependent brother/sister)
i)        Self attestation proforma (in case of children above 18 years of age excluding children with white cards)
j)       Receipt of MRO
k)      Bankers certificate
l)        Put the signature of all the beneficiaries in blue ink in a plain paper, scan and store in your pen drive.
m)    Scan the photographs of all the beneficiaries and keep ready in your pen drive.
n)      Scan your old smart card and store in your pen drive.

It is better you have a fresh photo as per the conditions of ECHS.  In case the face in the photo or signature is unclear, the application may be rejected. If the photographs/signatures on the Smart card at the time of presenting for treatment at ECHS Polyclinic does not match, the ECHS membership may be cancelled.  Be careful. Very strict military rules applied.

Note: Those who have lost their smart cards, those who are having temporary slips and those who wish to join fresh are to apply through on line now..  Those who are having 16kb cards have to apply before Sep 2018.  Those who are having 32kb cards can apply upto dec 2020.

 If you get all the above listed documents ready, you can apply easily.  You need not go anywhere except to a correct person who can upload your online application.  Your temporary slip will be generated online and you have to activate in your polyclinic.  Once your card is ready, you will receive SMS, then you have to persosnally visit the Station HQ to collect the Card.

Government of India issued instructions to abolish affidavits and attestation, but the CO ECHS is not bothered.  The Supreme Court orders not to insist Aadhaar for anything upto Mar 2018.  Again the CO ECHS  is not bothered.  Now a Sepoy to Hav have to pay Rs.30,000 to join ECHS and pay Rs.12000 every year for availing this ECHS service.  The ECH Scheme is not free scheme.  We are paying heavily and availing services under very strict military conditions.

The banks don’t bother to stop the FMA in spite of repeated request by the pensioners in writing.  Now the CO ECHS says that they will suspend the ECHS cards till the time all payments are refunded to the govt.  See how a nice service the ECHS is providing to its members.

Normally a smart card is costing only around 75 rupees only all over India.  But the ECHS asking us to pay Rs.177 for each member of a family. Within 13 years we are compelled to change the ECHS cards three times.  There is nobody to question this atrocity.  Why can’t they renew the cards freely?  Now for the online smart card application, every member has to spend atleast Rs.3000.  One 85 year old family pensioner has to apply for PAN Card.
