Every ESM/widows of Tamil Nadu who have not submitted Data Bank forms to Submit their Data Bank forms duly filled and signed to their dependent CSD Canteen with copy to OIC, ESM Helpline Centre, Fort St George, Chennai-09. The format available and submission of this form is compulsory and for clarifications contact Capt M David (Tele 044-25675236) on all working days between 0900hrs to 1500 hrs.
